CSB “Shock to the System” Video Released

This week, the US Chemical Safety and Hazard Investigation Board (CSB) released a short video detailing the events which triggered the hydraulic shock and ensuing ammonia release at Millard Refrigerated Services in the summer of 2010. Through helpful graphics and a step-by-step explanation of the scenario as it unfolded, the CSB highlights the safety lessons learned, demonstrating how catastrophic accidents such as this one can be prevented in the future.
